This Request for Supplier Qualifications (“RFSQ”) is an invitation by Wellington-Dufferin-Guelph Public Health (“WDGPH”) to prospective respondents to qualify in accordance with Evaluation of Responses (Part 2) for eligibility to provide Non-clinical and Clinical Renovation Construction Services as further described in Section A of the RFSQ Particulars (Appendix A) (the “Deliverables”).

WDGPH is a public health unit in Ontario with offices located in Guelph, Orangeville and Fergus.

WDGPH wishes to create a roster of prequalified General Contractors to provide the following renovation construction services for projects with an estimated budget of between $10,000.00 and $50,000.00, over the next three years.

a)  Renovation Construction Services in a Non-Clinical Environment 

b)  Renovation Construction Services in a Clinical Environment (Medical or Dental)

Only the prequalified General Contractors in each roster category will receive further notices of procurement covered by the roster.  

Specifically, within the Clinical Environment category, only those prequalified General Contractors who have provided a Dental project in Appendix B of the CCDC-11 2019 Contractor’s Qualification Statement will receive further notices for dental procurement covered by the roster. 

Should a prequalified General Contractor within the Clinical Environment category obtain experience with a similar Dental project AFTER prequalification for the Medical component of the category, the General Contractor may submit an additional Appendix B of the CCDC-11 2019 to the WDGPH Procurement Officer.  The additional Appendix B will be evaluated in accordance with the evaluation criteria specified in the Evaluation Criteria specified in Appendix A – RFSQ Particulars at the next quarterly review of submissions.

A General Contractor may submit a response for one or both categories.
